ABSTRACT

Introduction: The relationship between the tumor inflammatory infiltrate, also known as tumor-infiltrating lymphocytes (TILs), and invasive breast carcinomas has been extensively studied in recent years to verify its association with prognosis and response to treatment. The goal of this study was to associate the presence of TILs with patient's survival time. Methods: We studied prognostic clinicopathological characteristics already established in the literature and their impact on overall five-year survival time of patients with invasive breast cancer treated at Hospital Santa Casa in Belo Horizonte, Minas Gerais, Brazil, in 2011 (n=290). This was an observational and retrospective study. Results: The presence of TILs was associated with tumors of no special type (p=0.018) and with younger age of the patients (p=0.042). Smaller tumor size (HR: 19.24; 95%CI 4.30­86.15; p<0.001), absence of metastasis to the axillary lymph nodes (HR: 2.80; 95%CI 1.02­7.70; p=0.002), positivity for progesterone receptor (HR: 0.39; 95%CI 0.17­0.87; p=0.022), and presence of TILs (HR: 0.23; 95%CI 0.08­0.65; p=0.005) were associated with longer survival times. Conclusions: This study suggests that the presence of TILs, along with other clinicopathological characteristics, is a prognostic factor in breast cancer

ABSTRACT

Lymphonodopathy is an increase in volume and/or changes in the characteristics of lymph nodes, and it can be caused by benign or malignant diseases. Appropriate physical examination should define their clinical characteristics, and, if needed, complementary imaging or anatomopathological tests should be performed for diagnostic definition. In the present article, we report the case of a female patient, with sarcoidosis, who presented axillary lymph node disease, and the exams that followed until the diagnostic conclusion.

ABSTRACT

Whether age is an independent prognostic factor in breast cancer is a matter of debate. This is a retrospective cohort study of 767 breast cancer patients, stages I-III, treated at the Hospital das Clínicas, Minas Gerais Federal University, Belo Horizonte, Minas Gerais State, Brazil, from 2001 to 2008, aiming to study the relationship between age and survival. We included variables related to patients, tumors, and types of treatment. Different sets of Cox models were used for survival analysis. Hazard ratios (HR) and 95%CI were calculated. The relationship between age and breast cancer survival did not change substantially in any of them. In the model that accounted for all variables, women aged 70 and older (HR = 1.51, 95%CI: 1.04-2.18), and 35 or younger (HR = 1.78, 95%CI: 1.05-3.01) had shorter cancer specific survival than patients aged between 36 and 69. In addition, older age, having at least one comorbidity, and being white were associated with a higher risk of dying from other causes. In conclusion, shorter breast cancer survival is expected among the youngest and oldest patients.


É discutível se idade é um fator prognóstico independente para câncer de mama. Conduzimos uma coorte retrospectiva de 767 pacientes com câncer de mama, estádios I-III, tratadas no Hospital das Clínicas, Universidade Federal de Minas Gerais, Belo Horizonte, Minas Gerais, Brasil, de 2001 a 2008, para estudar a relação entre idade e sobrevida. Incluímos variáveis relacionadas às pacientes, aos tumores e ao tratamento. Diferentes conjuntos de modelos de Cox foram construídos. As razões de risco (RR) e IC95% foram calculados. A relação entre idade e sobrevida por câncer de mama não foi alterada substancialmente entre os modelos de Cox. No modelo com todas as variáveis explicativas, as mulheres de 70 anos ou mais (RR = 1,51; IC95%: 1,04-2,18) e até 35 anos (RR = 1,78; IC95%: 1,05-3,01) tiveram sobrevida causa-específica mais curta que as de 36-69 anos. Idades a partir de 70 anos, ter ao menos uma comorbidade e ser branca foram associadas a risco maior de óbito por outras causas. Em conclusão, as pacientes mais jovens e as mais idosas parecem ter sobrevida mais curta por câncer de mama.


Es discutible si la edad es un factor pronóstico independiente para el cáncer de mama. Se realizó sobre una cohorte retrospectiva de 767 pacientes con cáncer de mama, etapas I-III, atendidas en el Hospital de Clínicas, Universidad Federal de Minas Gerais, Belo Horizonte, Minas Gerais, Brasil, entre 2001 y 2008, para estudiar la relación entre edad y supervivencia. Incluimos variables relacionadas con las pacientes, los tumores y el tratamiento. Se construyeron diferentes conjuntos de modelos de Cox. Se calcularon los cocientes de riesgo (CR) e IC95%. La relación entre edad y supervivencia del cáncer de mama no ha cambiado substancialmente en los modelos. En el modelo con todas las variables, las mujeres de 70 años o más (CR = 1,51; IC95%: 1,04-2,18) y 35 años o menos (CR = 1,78; IC95%: 1,05-3,01) tuvieron menor supervivencia por cáncer de mama que las de 36 a 69 años. Tener edad avanzada, al menos una comorbilidad, y ser de piel blanca se asociaron a un mayor riesgo de morir por otras causas. En conclusión, las mujeres más jóvenes y las mayores parecen tener menor supervivencia de cáncer de mama.

ABSTRACT

O câncer de mama é a neoplasia maligna mais frequente nas mulheres, inclusive na pós-menopausa. O tratamento hormonal e quimioterápico dessa doença pode induzir o aparecimento de sintomas vasomotores, atrofia urogenital, alteração da função sexual e instalação da menopausa precocemente, os quais merecem avaliação e controle. O tratamento dos sintomas climatéricos em pacientes sobreviventes do câncer de mama tem grande relevância clínica, pois, em razão do aumento da incidência e dos avanços na abordagem terapêutica, o número de mulheres que sobrevivem ao câncer de mama e experimentam sintomas climatéricos tem aumentado. Objetivou-se identificar na literatura evidências recentes acerca das opções terapêuticas na redução dos sintomas climatéricos. Dessa forma, foi realizada uma revisão na literatura por meio de consulta nas principais bases de dados, priorizando artigos mais recentes e com maior nível de evidência. Os estudos randomizados controlados sobre o uso de fitoterápicos, acupuntura e ioga no tratamento de fogachos em pacientes com câncer de mama são limitados, tanto nas opções de tratamento e tempo de seguimento quanto na comprovação científica de sua eficácia. Abordando os tratamentos farmacológicos, vários inibidores da recaptação de serotonina (paroxetina, fluoxetina e citalopram) e inibidores da recaptação de serotonina e adrenalina (venlafaxina e desvenlafaxina) têm sido mais eficazes que o placebo na redução dos sintomas vasomotores em estudos a curto prazo em mulheres com câncer de mama. A avaliação dos sintomas e seu impacto na qualidade de vida, além do desejo da paciente, são determinantes na escolha do tratamento. Independentemente do tipo de tratamento indicado, as modificações de estilo de vida devem ser recomendadas.(AU)


Breast cancer is the most commonly diagnosed malignancy in women, including after menopause. The hormonal and chemotherapeutic treatment of this disease can induce the appearance of vasomotor symptoms, urogenital atrophy, abnormal sexual function and installation of early menopause that require evaluation and control. The treatment of climacteric symptoms in breast cancer survivors is of great clinical relevance, once the number of women experiencing menopausal symptoms has increased because of the high incidence of breast cancer and advances in therapeutic approach. The purpose is to identify recent evidence in the literature on the therapeutic options in reducing these menopausal symptoms. Thus, a literature review through consultation was held in the main databases, prioritizing newer and higher level of evidence items. Randomized controlled trials on the use of herbals, acupuncture and yoga in the treatment of hot flashes in patients with breast cancer are limited, such as treatment options, follow-up time and the scientific evidence of its effectiveness. Addressing the pharmacological treatments, several serotonin reuptake inhibitors (paroxetine, fluoxetine and citalopram) and reuptake inhibitors of serotonin and adrenaline (venlafaxine and desvenlafaxine) have been more effective than placebo in reducing vasomotor symptoms in short-term studies in women with breast cancer. The evaluation of symptoms and their impact on quality of life, beyond the desire of the patient, are determinant in the choice of treatment. Regardless of the type of treatment indicated, changes in lifestyle should be recommended.(AU)

CONTEXT: Intramuscular lipomas are benign tumors that infiltrate the muscles. CASE REPORT: We describe the case of a 58-year-old female patient with an axillary lump. The lump was a lipoma inside the subscapularis muscle. It is important to differentiate these lesions from liposarcomas and from other diseases that may present as axillary lumps. The most accurate imaging method for differentiating benign lipomatous tumors from liposarcomas is magnetic resonance imaging, but surgical removal of these intramuscular lesions to confirm the diagnosis is recommended. CONCLUSION: Intramuscular lipomas are a rare cause of benign axillary lumps and should be considered in making differential diagnoses on axillary masses. .


CONTEXTO: Lipomas intramusculares são tumores benignos que infiltram os músculos. RELATO DE CASO: Descrevemos o caso de uma paciente de 58 anos com nódulo axilar. O nódulo era um lipoma na intimidade do músculo subescapular. É importante diferenciar essas lesões de lipossarcomas e outras doenças que podem acometer a axila. O método de imagem mais eficaz para diferenciar lesão lipomatosa benigna do lipossarcoma é a ressonância magnética, mas é recomendada a remoção cirúrgica dessas lesões intramusculares para confirmar o diagnóstico. CONCLUSÃO: Lipomas intramusculares são causas raras de nódulos axilares benignos e devem ser considerados no diagnóstico diferencial dessas lesões. .

ABSTRACT

O câncer de mama é a neoplasia maligna mais frequente em mulheres tanto no Brasil quanto no mundo. A doença é mais comum acima dos 50 anos, coincidindo com a faixa etária de risco para o câncer de endométrio. O tamoxifeno é um modulador seletivo de receptor de estrogênio (SERMs), usadona terapêutica das mulheres portadoras de câncer de mama. Assim como os outros SERMs (raloxifeno,toremifeno,arzoxifeno e lasoxifeno), o tamoxifeno pode atuar como antagonista ou agonista, dependendo do tecido-alvo.Nestas pacientes, o uso destes agonistas seletivos embora apresente maior benefício do que risco para o tratamento do câncer de mama, pode causar efeitos secundários no endométrio, com aumento do risco para doenças malignas. Consensos atuais, porém, não demonstram benefício de nenhum método de rastreio para câncer endometrial de rotina. O que se recomenda, nas pacientes na pré e pós-menopausa com câncer de mama, é o exame ginecológico com intervalo anual e o prosseguimento com propedêutica, através de biópsia do endométrio nas pacientes pós-menopausa que apresentam sangramento vaginal.(AU)


Breast cancer is the most common malignancy in women both in Brazil and in the world. The disease is more common over 50 years, coinciding with the age of risk for endometrial cancer. Tamoxifen is a selective modulator of estrogen receptor (SERMs) used in the treatment of women with breast cancer. Like other SERMs (raloxifene, toremifene, arzoxifeno and lasoxifeno), tamoxifen may act as antagonist or agonist depending on the target tissue. In these patients, although showing greater benefit of what risk for the treatment of the breast cancer, can cause side effects on the endometrium, with increased risk for malignant diseases. Current consensus, however, do not demonstrate any benefit from routine screening method for endometrial cancer. Women with breast cancer should undergo annual gynecologic examinations for premenopausal and postmenopausal patients and further workup by means of biopsy in patients with postmenopausal vaginal bleeding.(AU)

ABSTRACT

OBJECTIVE: To determine the frequency of the immunohistochemical profiles of a series of high-grade ductal carcinoma in situ of the breast. METHODS: One hundred and twenty-one cases of high-grade ductal carcinoma in situ, pure or associated with invasive mammary carcinoma, were identified from 2003 to 2008 and examined with immunohistochemistry for estrogen receptor, human epidermal growth factor receptor 2, cytokeratin 5, and epidermal growth factor receptor. The tumors were placed into five subgroups: luminal A, luminal B, HER2, basal-like, and “not classified”. RESULTS: The frequencies of the immunophenotypes of pure ductal carcinoma in situ were the following: luminal A (24/42 cases; 57.1%), luminal B (05/42 cases; 11.9%), HER2 (07/42 cases; 16.7%), basal-like phenotype (00/42 cases; 0%), and “not classified” (06/42 cases; 14.3%). The immunophenotypes of ductal carcinoma in situ associated with invasive carcinoma were the following: luminal A (46/79 cases; 58.2%), luminal B (10/79 cases; 12.7%), HER2 (06/79 cases; 7.6%), basal-like (06/79 cases; 7.6%), and “not classified” (11/79 cases; 13.9%). There was no significant difference in the immunophenotype frequencies between pure ductal carcinoma in situ and ductal carcinoma in situ associated with invasive carcinoma (p>0.05). High agreement was observed in immunophenotypes between both components (kappa=0.867). CONCLUSION: The most common immunophenotype of pure ductal carcinoma in situ was luminal A, followed by HER2. The basal-like phenotype was observed only in ductal carcinoma in situ associated with invasive carcinoma, which had a similar phenotype. .

ABSTRACT

OBJETIVO: Avaliar a concordância nos diagnósticos histopatológicos de lesões mamárias proliferativas intraductais entre patologistas gerais e especialistas em patologia mamária. MÉTODOS: Trata-se de estudo observacional e transversal, com análise de 209 lesões encaminhadas ao Laboratório de Patologia Mamária da Faculdade de Medicina da Universidade Federal de Minas Gerais para consultoria, no período de 2007 a 2011, comparando os diagnósticos originais com os após a revisão. Foram incluídos apenas os casos com solicitação formal de revisão e que apresentavam diagnóstico histopatológico no laudo original ou de revisão de lesões proliferativas, carcinoma ductal in situ puro, carcinoma ductal in situ com microinvasão ou associado a carcinoma invasor. A concordância percentual e o índice kappa foram utilizados para a análise estatística. RESULTADOS: Observamos moderada concordância nos diagnósticos originais de benignidade ou malignidade versus os diagnósticos de revisão (kappa=0,5; concordância percentual=83%). Após a revisão, o diagnóstico de malignidade foi confirmado em 140/163 casos (86%) e o diagnóstico de benignidade foi confirmado em 34/46 casos (74%). Quanto aos diagnósticos específicos, observamos concordância moderada entre o laudo original e de revisão (136/209 casos; kappa=0,5; concordância percentual=65%). A maior discordância foi observada nos casos de carcinoma ductal in situ com microinvasão (6/6 casos; 100%). Grande discordância foi observada nos casos de hiperplasia ductal atípica (16/30 casos; 53%) e carcinoma ductal in situ (25/75 casos; 33%). Em relação ao grau histológico do carcinoma ductal in situ, observou-se boa concordância entre os laudos originais e de revisão (29/39 casos; kappa=0,6; concordância percentual=74%). CONCLUSÃO: Nossos dados confirmam que as lesões mamárias proliferativas intraductais, em especial as hiperplasias ductais atípicas, o carcinoma ductal in situ e o carcinoma ductal in situ com microinvasão apresentam relevantes discordâncias nos diagnósticos histopatológicos, que podem induzir o clínico a erros nas decisões terapêuticas.


PURPOSE: To evaluate the agreement about the histopathological diagnosis of intraductal proliferative breast lesions between general pathologists and a specialist in breast pathology. METHODS: This was an observational, cross-sectional study of 209 lesions received in consultation at the Breast Pathology Laboratory of the School of Medicine, Federal University of Minas Gerais, from 2007 to 2011, comparing the original diagnosis and the review. We included only cases with a formal request for review and cases in which the original diagnosis or reviewer's diagnosis showed proliferative lesions, pure ductal carcinoma in situ, ductal carcinoma in situ associated with microinvasion or associated with invasive carcinoma. The kappa index and percent concordance were used in the statistical analyses. RESULTS: A moderate agreement was observed between the original histopathological diagnosis and the second opinion (kappa=0.5; percentual concordance=83%). After the review, the diagnosis of malignancy was confirmed in 140/163 cases (86%) and the diagnosis of benign lesions was confirmed in 34/46 cases (74%). Regarding specific diagnosis, we observed moderate agreement between the original diagnosis and the reviewer's diagnosis (136/209 cases; kappa=0.5; percent concordance=65%). The highest disagreement was observed in cases of ductal carcinoma in situ with microinvasion (6/6 cases; 100%). Important discordance was observed in cases of atypical ductal hyperplasia (16/30 cases; 53%) and ductal carcinoma in situ (25/75 cases; 33%). Regarding the histological grade of ductal carcinoma in situ, we observed good agreement between the original diagnosis and the review (29/39 cases; kappa=0.6, percent agreement=74%). CONCLUSION: Our data confirm that intraductal proliferative breast lesions, especially atypical ductal hyperplasia, ductal carcinoma in situ and ductal carcinoma in situ with microinvasion show relevant discrepancies in the histopathological diagnoses, which may induce errors in therapeutic decisions.

CONTEXT AND OBJECTIVE: Recently, breast-conserving surgery (BCS) has been replacing mastectomy for breast cancer treatment. The aim of this study was to evaluate the changes in mastectomy and BCS rates and the factors relating to these shifts. DESIGN AND SETTING: A retrospective study in a Brazilian public hospital. METHODS: Pathological records from female patients who underwent surgery for breast cancer at Hospital das Clínicas, Universidade Federal de Minas Gerais (HC-UFMG), between 1989 and 2008 were reviewed. The mastectomy and BCS rates were calculated. The chi-square test was used to assess factors associated with type of surgical treatment and to compare trends in treatment type over the years. Logistic regression was used for multivariate analysis. RESULTS: From 1989 to 2008, 2050 breast cancer surgical specimens were received in our service, corresponding to 1973 patients; 1324 (64.6%) of them were from mastectomy and 726 (35.4%) from BCS. A shift from mastectomy towards BCS was observed (P < 0.001). In multivariate analysis, earlier year of surgery (P < 0.001), larger tumor size (P < 0.001), having at least one positive axillary lymph node (P < 0.001) and patients' age greater than 68 years (P = 0.007) were predictors of mastectomy. CONCLUSIONS: There was a shift from mastectomy towards BCS in our institution over the years. This may reflect consolidation of BCS (plus radiotherapy) as an equivalent treatment to mastectomy in terms of survival and a shift to earlier diagnosis for the disease.


CONTEXTO E OBJETIVO: Recentemente, a cirurgia conservadora tem substituído a mastectomia para tratamento cirúrgico do câncer de mama. O objetivo deste estudo é avaliar as mudanças nas taxas de mastectomia e cirurgia conservadora e os fatores relacionados a essas mudanças. TIPO DE ESTUDO E LOCAL: Estudo retrospectivo em hospital público brasileiro. MÉTODOS: Dados patológicos das pacientes submetidas a cirurgia para câncer de mama no Hospital das Clínicas, Universidade Federal de Minas Gerais (HC-UFMG), de 1989 a 2008, foram revistos. As taxas de mastectomia e cirurgia conservadora foram calculadas. O teste qui-quadrado foi usado para avaliar fatores relacionados ao tipo de tratamento cirúrgico empregado e comparar tendências de tipo de tratamento ao longo dos anos. Regressão logística foi usada para análise multivariada. RESULTADOS: De 1989 a 2008, 2.050 espécimes cirúrgicos de câncer de mama foram recebidos em nosso serviço, correspondentes a 1.973 pacientes; 1.324 (64,6%) correspondentes a mastectomia e 726 (35,4%) correspondentes a cirurgia conservadora. A cirurgia conservadora se tornou mais frequente ao longo dos anos (P < 0,001). Em análise multivariada, ano mais precoce da cirurgia (P < 0,001), maior tamanho tumoral (P < 0,001), ter ao menos um linfonodo axilar positivo (P < 0,001) e idade maior de 68 anos (P = 0,007) foram preditores de mastectomia. CONCLUSÕES: As cirurgias conservadoras se tornaram mais frequentes em relação às mastectomias ao longo dos anos. Isso pode refletir a consolidação da cirurgia conservadora (associada a radioterapia) como equivalente à mastectomia em termos de sobrevida, bem como diagnóstico mais precoce da doença.

ABSTRACT

OBJETIVO: Fatores genéticos e ambientais são conhecidos por sua importância na gênese de grande parte das doenças. É possível estudá-los pela observação da prevalência de agravos nas populações, bem como das causas de mortalidade, buscando-se uma correlação com os hábitos sociais e origens étnicas e familiares dos indivíduos. Nestes estudos, destacam-se as comunidades nas quais podem-se verificar vínculos sociais e genéticos entre seus membros. A comunidade judaica de Belo Horizonte se encaixa nesses critérios. Ela se consolidou na década de 20 do século passado e, atualmente, a Federação Israelita de Minas Gerais (FISEMG) tem cadastro de aproximadamente 600 famílias. Este estudo pretende definir o padrão de mortalidade nessa comunidade, e como ele se modificou a partir de sua formação. MÉTODOS: Foram recuperados dados a partir dos arquivos do Instituto Histórico Israelita Mineiro, da FISEMG e dos Cemitérios Israelita e do Bonfim, oriundos de registros históricos e guias de sepultamento, no período de 1926 a 2003. RESULTADOS: Foram estudados 601 registros de óbitos, sendo 61,6 por cento de homens e 38,4 por cento de mulheres. As doenças infecto-parasitárias, dentre elas a tuberculose, ocorreram com maior freqüência nas décadas de 30 e 40 e decresceram nas décadas seguintes. As doenças do aparelho circulatório foram as causas de óbito mais prevalentes a partir da década de 40. CONCLUSÃO: Houve mudança nos padrões de mortalidade da população judaica de Belo Horizonte.


OBJECTIVE: Genetic and environmental factors are known for their importance in the genesis of the majority of diseases. It is possible to study them through the observation of prevalence of diseases, and also the mortality causes, relating it to social habits and ethnical and familial origins of the individuals. In those studies, communities in which there are social and genetic links stand out. The jewish community of Belo Horizonte fits these criteria. It has been consolidated in the 20th decade of the last century. Nowadays the Federação Israelita de Minas Gerais (FISEMG) has records of approximately 600 families. The purpose of this study is to define the mortality pattern in this community, and how it has changed since the community's formation. METHODS: Data was recovered from the files of the Instituto Histórico Israelita Mineiro, the FISEMG and from the cemeteries Israelita and Bonfim. RESULTS: 601 death registries have been studied, 61,6 percent belonging to men and 38,4 percent to women. The diseases of the circulatory system were the most frequent cause of death since the decade of 40, and infectious diseases, including tuberculosis, deserve note in the decades of 30 and 40. CONCLUSION: There was a change in death pattern of the jewish community of Belo Horizonte.
